Community Health Nurse

What is a Community Health Nurse (CHN)
A Community Health Nurse is a specialist field for Division 1 Registered Nurses.  Community health nurses undertake special education to develop their skills of working with people the community settings.  They focus on working with people to reduce their risks of getting sick (primary prevention).  A community health nurse will also work with people to manage any conditions that might already exist; to stop those getting worse (secondary prevention)

How to access the CHN?
To access the CHN you can telephone (03) 58 233 200 or call in to 399 Wyndham Street, Shepparton.   To make an e-mail referral you can select the e-referral link at the bottom of this page; this will be forwarded to our Central Intake Worker who will make contact with you to arrange a time to come in.      

CHN Hours:
Monday – 9am to 5pm at Goulburn Valley Community Health Service 399 Wyndham Street, Shepparton
Tuesday – 9am to 5pm at Goulburn Valley Community Health Service 399 Wyndham Street, Shepparton
Wednesday – 9am to 5pm at Goulburn Valley Community Health Service 399 Wyndham Street, Shepparton

What does a CHN do?
A CHN provides a range of health promotion, education activities and direct client services to people, with the aim of raising the general level of health and wellbeing of persons within the community.  The CHN can work with individuals or groups of people.  Asthma management, cancer prevention and/or living with a cancer diagnosis are some of the health issues of particular importance; however the CHN is able to work with people on a variety of health related issues.

Where is the service provided?
At Goulburn Valley Community Health Service, 399 Wyndham Street, Shepparton.  The CHN can also visit people in their homes or workplaces.

Who can access the CHN?
Any person can access the CHN, regardless of where they live.  People who want to visit the CHN do not need to have a Health Care Card.

What the CHN is not:
The CHN does not provide the same service as a District Nurse.  The CHN does not re-dress wounds, give injections or prescribe medicines.
