The Intake Worker is the first point of contact with our agency and is responsible for the provision of information, resources and/or access to services via the process of Initial Needs Identification.

Chronic Conditions Self-Management Worker - Readvertised
(0.7 EFT)
The Chronic Conditions Self-Management (CCSM) program targets people with chronic disease. The worker will provide support for people with a diagnosed chronic disease and complex needs focusing on people who are assessed as at risk of progressing towards hospitalisation and requiring a managed and planned approach to reduce risk.

Alcohol and Other Drugs Youth Outreach Worker
(1 EFT)
The Youth Outreach Worker positions aims to provide support to young people with problematic alcohol and/or other drug use aged 10 – 25 years. You will also be responsible for the provision of early intervention and community education initiatives.

Alcohol and Other Drugs Counsellor
(1 EFT)
Supported by a team of qualified professionals, you will be responsible for the provision of evidence based clinical services including assessment, case management, counselling and group work for clients with identified alcohol and other drug issues.
